Order Genome Sequencing

The Genome Sequencing test incorporates the power of genome sequencing with rigorous clinical interpretation of sequence information. The Genome Sequencing test will provide physicians with unparalleled interpretation of genomic data, and aims to end the diagnostic odyssey for patients with rare genetic disorders. Read more details about this test.

Requisition Form

All samples must be accompanied with a completed requisition form. Please make sure any identifiers used on the specimen are provided on the paperwork. Consent page should be signed by a health care provider. Any incomplete or missing paperwork may delay the start of testing.
